Common Knee Problems

Patellofemoral Pain Syndrome (PFPS)

Symptoms: Pain at the front of the knee, worsened when climbing or descending

Setup causes: Saddle too low, incorrect cleat position

Prevention: Correct saddle height (25-35 degrees), cleat ball of foot 5-10 mm ahead of the pedal axle

IT Band Syndrome

Symptoms: Pain on the outside of the knee, starting after 15-20 minutes of riding

Causes: Weak gluteal muscles, incorrect cleat angle

Prevention: Glute strengthening exercises, cleat adjustment

Strengthening Exercises

  • Straight leg raises: 3 × 12 (per leg)
  • Glute bridges: 3 × 15
  • Side-lying clamshells: 3 × 15 (per side)

Flexibility Exercises

  • Quadriceps stretch: 30 seconds × 3
  • Hamstring stretch: 30 seconds × 3
  • IT band stretch: 30 seconds × 3 (per side)

Riding Recommendations

  • Maintain cadence of 85-100 RPM
  • Use pedals with float
  • Avoid low cadence with high resistance

Recovery Plan

Weeks 1-2: 5-10 minutes of easy riding
Weeks 3-4: Increase to 15-20 minutes
Weeks 5-8: Increase to 30-45 minutes
