Knee Pain: The Most Common Cycling Injury

Anterior Knee Pain (Front of the Knee)

Causes:

  • Saddle too low (most common cause)
  • Knee excessively tracking forward past the toes during pedaling
  • Incorrect cleat angle settings

Symptoms: Pain around or below the kneecap after climbing or long rides (patellar tendinitis or chondromalacia patella)

Prevention and Improvement:

  1. Raise the saddle: When your heel is on the pedal, your knee should be fully extended; when using the normal pedaling position on the pedal, your knee should retain a bend of approximately 25–30 degrees (at the lowest point)
  2. Cleat float: Choose a cleat system with float angle to allow a small degree of rotation for the ankle
  3. Increase pedaling cadence: Raise cadence to 85–95 rpm to reduce torque per pedal stroke

Posterior Knee Pain (Back of the Knee)

Causes:

  • Saddle too high (most common cause)
  • Insufficient hamstring flexibility

Prevention and Improvement:

  1. Ensure the saddle height is not too high (knee bend of approximately 25–30 degrees at the lowest point of the pedal stroke)
  2. Strengthen hamstring flexibility through stretching

Lateral Knee Pain (Iliotibial Band Syndrome)

Causes:

  • Sudden large increase in riding volume
  • Pedal position too wide
  • Excessive outward pedaling force (influenced by X-legs or O-legs)

Prevention and Improvement:

  1. Progressively increase training volume (no more than 10% per week)
  2. Use a foam roller to massage the outer thigh
  3. Adjust the pedal Q-Factor (pedal spacing)

Neck Pain: The Nightmare of Long Rides

Cause Analysis

The forward-leaning riding position of road bikes requires the neck to be extended upward for long periods, causing the trapezius and posterior neck muscles to sustain isometric contractions continuously. Over time, this leads to stiffness and pain.

Combined with the inconsistent quality of Taiwan’s roads, vibrations travel through the fork to the handlebars and then to the neck, further increasing the burden.

  3. Vary hand positions: On flat roads, move your hands from the drops to the tops to change the neck angle
  4. Mobilize the neck during every ride: Perform neck rotations and side bends at red lights; don’t wait until pain occurs to move

Wrist and Palm Numbness: “Cyclist’s Palsy”

Cause Analysis

While riding, the hands support the upper body’s weight, causing continuous compression on the ulnar nerve and median nerve, leading to palm numbness. In severe cases, it can even affect fine motor skills in daily life.

Solutions

  1. Thicker handlebar tape: Choose handlebar tape with adequate cushioning; gel materials are effective
  2. Cycling gloves: Use gloves with a padded layer
  3. Adjust handlebar position: The handlebars should not force excessive wrist bending; keep the wrist close to a neutral position
  4. Vary hand positions: Road bikes offer multiple hand positions—make good use of them
  5. Reduce upper body weight distribution: Strengthen the core muscles to reduce the load on the hands

Overall Prevention Principles

The Importance of Bike Fit

Most of the pains mentioned above can be prevented through proper bike setup (Bike Fit). A professional bike fit costs approximately NT$2,000–5,000, but it can prevent tens of thousands in medical expenses and weeks of lost riding time.

It is recommended to seek a Bike Fit in the following situations:

  • After purchasing a new bike
  • When experiencing any unexplained riding discomfort
  • Before significantly increasing training volume

The Principle of Progression

80% of cycling injuries result from “increasing training volume too quickly.” Weekly mileage should not increase by more than 10% from the previous week, and every 4 weeks should include a “recovery week” (reducing training volume by 20–30%).

Listen to Your Body

“Riding through the pain” is a dangerous culture among cyclists. Minor discomfort can be ridden through while monitoring it, but you should stop immediately and seek medical attention in the following situations:

  • Knee swelling or localized heat
  • Pain at rest during the night
  • Pain that continues to worsen during riding (rather than easing after a proper warm-up)
