The unique posture of cycling places considerable static load on the neck and shoulders:

  • Riders need to keep their heads up and look forward for extended periods (cervical extension)
  • The upper body leans forward, and the weight of the head (about 5-6 kg) must be supported by the neck muscles
  • Hands are supported on the handlebars, keeping the shoulders under continuous static load

The Special Role of the Upper Trapezius

The Upper Trapezius is a triangular muscle extending from the base of the skull to the collarbone and shoulder blade. Its main functions are:

  • Elevating the shoulder blade (shrugging motion)
  • Assisting with lateral neck flexion and rotation
  • Stabilizing the shoulder blade during shoulder elevation

1. Upper Trapezius Tightness and Trigger Points

2. Cervical Myofascial Pain Syndrome

Widespread pain involving multiple neck muscle groups, accompanied by reduced range of motion, headaches, and sometimes numbness in the arms.

3. Thoracic Outlet Syndrome (TOS)

Maintaining a forward-leaning posture for long periods can compress the passage beneath the collarbone, causing arm numbness, tingling, especially in the fourth and fifth fingers. If you experience these symptoms, you should seek medical evaluation.

4. Cervical Disc Problems

The cervical spine pressure from prolonged riding posture may accelerate pre-existing cervical degeneration, triggering nerve root symptoms.

The Impact of Bike Fit on the Neck and Shoulders

Handlebar Height (Most Critical)

The lower the handlebars, the greater the neck extension angle required to maintain forward vision, and the greater the load on the upper trapezius.

Recommendation: Riders with neck and shoulder issues should consider raising the handlebar height (by increasing the stem height or using a riser stem).

Stem Length

A stem that is too long overextends the body forward, increasing static load on the neck and shoulders.

Recommendation: Appropriately shorten the stem length to bring the body closer to the handlebars and reduce the need to reach forward.

Handlebar Width

Handlebars that are too narrow cause the shoulders to roll inward, increasing pressure on the upper trapezius and rhomboids; handlebars that are too wide require greater shoulder abduction to maintain posture.

General Recommendation: Handlebar width should be equal to or slightly narrower than shoulder width.

Relief and Treatment Strategies

Immediate Relief (During Riding)

Perform the following movements every 20-30 minutes (can be done on easy sections of the ride):

  1. Make large backward circles with the shoulders 10 times
  2. Slowly turn the neck left and right, 5 times each direction
  3. Release one hand from the handlebars and perform a shoulder stretch (do this carefully and safely)
  4. Occasionally change hand positions (from the drops to the hoods)

Post-Ride Stretching (Very Important)

Perform the following stretches after every ride, holding each movement for 30 seconds × 2-3 sets:

  1. Neck Lateral Flexion Stretch:

    • Sit upright, bring your right hand over the top of your head to gently hold your left ear
    • Gently tilt your head toward the right, feeling a stretch on the left side of your neck
    • Repeat on the other side
  2. Scalene Stretch:

    • Tilt your head to the right and slightly tilt it back
    • Use your right hand to gently assist in deepening the stretch
  3. Upper Trapezius Stretch:

    • Tilt your head to the right, bringing your right ear toward your right shoulder
    • Simultaneously reach your left arm downward (pulling it toward the floor), increasing the stretch
  4. Chest Stretch (Chest Opener):

    • Interlace your fingers behind your back
    • Squeeze your shoulder blades together and push your chest upward
    • Counteracts the forward shoulder posture from riding

Strength Training (The Fundamental Solution)

Strengthen the Deep Cervical Flexors:
Weakness in the deep cervical flexors is one of the root causes of upper trapezius overuse. You can perform “Chin Tuck” training:

  • Sit or lie down
  • Gently draw your chin inward (a “double chin” motion)
  • Hold for 10 seconds, repeat 10 times, 3 sets per day

Mid and Lower Trapezius Strengthening:

  • Prone Y/T/W exercises
  • Rowing movements (with resistance bands or machines)
  • Reverse flys

Rhomboid Strengthening:

  • Horizontal rows with resistance bands
  • Scapular retraction exercises

Best Strategies for Prevention

  1. Core Strength: A strong core keeps the upper body more stable while riding, reducing compensatory load on the neck and shoulders
  2. Proper Bike Fit: Get regular Bike Fits, especially before changing bikes or long-distance races
  3. Daily Maintenance Stretching for the Neck and Shoulders: No more than 10 minutes, but it should be done consistently
  4. Swimming or Water-Based Training: As supplementary training, it strengthens the shoulder and back muscles while reducing neck pressure
  5. Active Relaxation While Riding: Consciously and regularly “relax” your shoulders (avoid keeping them shrugged and tense)
