Cycling and the Knee
Cycling is a low-impact sport, but the highly repetitive pedaling motion (approximately 5,000-6,000 revolutions per hour) means that any minor biomechanical issue is amplified. Knee pain is the most common overuse injury among cyclists, but the good news is—most causes are preventable and correctable.
Types and Causes of Knee Pain
Anterior Knee Pain (Front of the Kneecap)
Common Causes:
- Saddle too low
- Saddle too far forward
- Gear ratio too heavy (low cadence)
- Incorrect cleat position
- Tight quadriceps
Solutions:
- Raise the saddle 5-10mm
- Move the saddle back 5-10mm
- Use a lighter gear, maintaining 80+ RPM
- Check cleat position
- Stretch the quadriceps before and after riding
Posterior Knee Pain (Back of the Knee)
Common Causes:
- Saddle too high
- Saddle too far back
- Insufficient hamstring flexibility
- Overextension
Solutions:
- Lower the saddle 5-10mm
- Move the saddle forward
- Increase hamstring stretching
- Avoid sudden increases in training volume
Lateral Knee Pain (Iliotibial Band Syndrome - ITBS)
Common Causes:
- Weak gluteus medius
- Incorrect cleat angle
- Pedaling with toes in or out
- Sudden increase in training volume
Solutions:
- Strengthen the gluteus medius (clamshells, lateral step-downs)
- Adjust cleat angle
- Use float cleats (increasing foot range of motion)
- Foam roll to release the IT band
Medial Knee Pain
Common Causes:
- Cleats causing excessive toe-out foot position
- Q-factor (distance between pedals) too wide or too narrow
- Flat feet issues
Solutions:
- Adjust cleat angle
- Consider using insoles to correct arch support
- Professional bike fitting assessment
Prevention Principles
1. The 10% Rule
Do not increase weekly training volume (distance or time) by more than 10%.
2. Maintain Appropriate Cadence
- Flat terrain: 85-95 RPM
- Climbing: no lower than 70 RPM
- Avoid prolonged heavy-gear, low-cadence grinding
3. Strength Training
2-3 sessions per week of lower-body strength training:
- Squats
- Lunges
- Single-leg deadlifts
- Glute bridges
- Clamshells
4. Flexibility
Stretch the following muscle groups after riding:
- Quadriceps
- Hamstrings
- Iliotibial band
- Calves
- Glutes
5. Proper Bike Fitting
- Get a fitting check once a year
- Reconfirm settings after changing components
- Check settings immediately if pain occurs
When Should You See a Doctor?
- Pain persists for more than 2 weeks and adjustments have not helped
- Swelling or heat
- Unable to walk normally
- A feeling of catching or locking in the knee
- Popping sounds accompanied by pain
