Most running instruction focuses on vertical oscillation and foot strike patterns, yet rarely mentions another issue that quietly steals energy—Lateral Sway / Lateral Trunk Motion. Every time your foot lands, if your center of mass deviates from the direction of travel, the extra transverse movement is equivalent to “taking the long way around.” Imagine a boat advancing while swaying side to side, versus one sailing steadily and straight—which is more efficient?

What Is Running Lateral Sway?

Lateral sway refers to the amplitude of movement of the pelvis and torso in the left-right direction (frontal plane) while running. This movement cannot be entirely eliminated (slight lateral movement is part of a natural gait), but excessive lateral sway can cause:

  • Increased unnecessary lateral kinetic energy
  • A longer actual travel path with each step
  • Increased compensatory load on the gluteus medius and core
  • Elevated risk of iliotibial band syndrome (ITBS)

Causes of Lateral Sway

Methods to Quantify Lateral Sway

  1. Rear-view video analysis: Have someone film your running form from directly behind. Observe the amplitude of horizontal head movement—ideally, the head should move almost straight forward, with lateral deviation no more than 2–3 cm.

  2. Treadmill + full-length mirror: On a treadmill, observe yourself from the front in a mirror. Check whether your shoulders rise and fall significantly side to side, and whether your head position stays centered.

  3. Wearable devices: Some Garmin and Polar devices can measure “Ground Contact Time Balance.” A large difference in ground contact time between the left and right foot typically reflects lateral asymmetry.

  4. Footprint test: Run a stretch on sand or snow, then observe whether the footprints form a nearly straight line or a distinct zigzag pattern.

Common Signs of Excessive Lateral Sway

  • Shoulders rise and fall significantly side to side while running (not front-to-back swinging)
  • The head appears to be nodding, but the direction is lateral
  • Companions watching you from behind say you look like you’re “dancing”
  • Persistent discomfort in the iliotibial band or outer knee after runs
  • One side of the hip or lower back tires or aches easily

Corrective Training Plan

The following exercises target the root causes of lateral sway:

Gluteus medius strengthening (3 times per week)

  1. Clamshell: Lie on your side with knees bent at 90 degrees, keep heels touching, and slowly lift the top knee. 3 sets × 15 reps.
  2. Monster Walk: Place a resistance band above the knees, take a half-squat stance, and step sideways. 20 steps per side, 3 sets.
  3. Single-Leg Glute Bridge: Lie on your back with one foot on the ground and the other leg raised, drive the hips up with force. 3 sets × 12 reps.

Core lateral stability training

  1. Side Plank: Lie on your side supported by your elbow and the side of your foot, keeping the body straight. Hold 30–45 seconds per side, 3 sets.
  2. Suitcase Carry: Hold a dumbbell in one hand (10% of body weight), walk 30 meters while keeping the torso from leaning.

Running form correction

  1. Step width awareness training: Run along the lane line on a track, attempting to land your feet close to but not crossing the line, to feel the correct step width (shoulder width or slightly narrower).
  2. Resistance band running: Run with a light resistance band above the knees, forcing the gluteus medius to actively engage. Perform for 5 minutes during the warm-up portion of each session.

Lateral Sway and Running Shoe Selection

Stability running shoes (Motion Control or Stability types) can help control excessive pronation to a certain degree, indirectly reducing lateral sway. However, shoes are only an auxiliary measure—the fundamental improvement still comes through strength training. If you notice a significant reduction in lateral sway after switching shoes, it may indicate that muscle weakness is the core issue.

Conclusion

Lateral sway is one of the most overlooked aspects of running efficiency. It is not as visually obvious as vertical bouncing, yet it quietly drains your energy and accumulates injury risk. The good news is that the return on investment from gluteus medius and core stability training is extremely high—even adding just two targeted strength sessions per week, most runners can feel a noticeable improvement in “stability” while running within six to eight weeks. And that stability is the foundation of efficiency.
