The cardiovascular system adapts quickly (2–4 weeks), far faster than tendons and cartilage (6–12 weeks). This time gap is the root cause of the most common injuries among ultra-endurance athletes.

Every year, thousands of cyclists in Taiwan suffer from Achilles tendinitis or Iliotibial Band Syndrome (ITBS) because their “passion exceeds their body’s capacity.” Mild cases require 1–2 weeks of rest; severe cases require 2–3 months of physical therapy. Worse, both injuries share a common trait: “early symptoms are subtle, and you only notice them when the pain becomes too severe to ride.”

Anatomical Basics: Know Your Vulnerable Points

Achilles Tendon:

  • The strongest tendon in the body, connecting the calf muscles (gastrocnemius and soleus) to the heel bone

  • Early signs: stiffness in the heel on the first step after waking up in the morning; tightness in the back of the ankle at the start of a ride

Iliotibial Band (IT Band):

  • A band of connective tissue extending from the pelvis (ilium) along the outside of the thigh to the tibia
  • Injury mechanism while cycling: saddle too high (causing the knee to repeatedly rub against the IT Band at the dead spot angle), cadence too low, excessive toe-in pedaling
  • Early signs: dull pain on the outside of the knee after riding; pain on the outside of the knee when climbing or descending stairs

  • After every 3–4 weeks of increased volume, schedule one recovery week (reduce volume back to pre-increase levels)
  • When early warning signs appear in the tendons, immediately reduce training volume by 30% and pause high-intensity workouts

Active Prevention Measures

Preventive Training for the Achilles Tendon:

Eccentric Calf Raises are currently the most scientifically supported method for preventing Achilles tendon injuries:

  1. Stand on the edge of a step with your heels hanging off
  2. Rise up on both feet (concentric), then lower slowly on one foot (eccentric, about 3–4 seconds)
  3. 15 repetitions per set, 3 sets per session, 3 days per week

Preventive Training for the IT Band:

  • Side-lying hip abduction (Clamshell Exercise): strengthens the gluteus medius, reducing tension on the IT Band
  • Foam roller massage on the outer thigh: perform for 3–5 minutes after each ride (note: this is for prevention before symptoms appear; do not directly press on the IT Band once symptoms are present)
  • Hip flexor stretch: perform after riding, 30 seconds per side, 3 sets each

  • Saddle height: A saddle that is too high is a common risk factor for both IT Band and Achilles tendon injuries. Standard: at the bottom of the pedal stroke, the knee should have approximately 20–25 degrees of flexion (not fully extended)
  • Cleat position: Cleats should be positioned directly under the metatarsal heads (ball of the foot); positioning them too far forward (toward the toes) significantly increases tension on the Achilles tendon

  1. Morning test: Is your heel stiff on the first step every morning? If this occurs for 3 consecutive days and worsens after riding, immediately reduce training volume and consult a physical therapist
  2. Pay special attention on descents: Long descents on Taiwan’s mountain roads are a high-risk activity for the IT Band. Maintain a higher cadence on descents and use braking appropriately (do not “lock the pedals” keeping the knee fixed in a bent position)
  3. Physical therapy resources in Taiwan: Many physical therapists familiar with sports injuries are available in major cities across Taiwan. Early mild symptoms can usually be improved in 2–4 sessions—do not wait until the condition becomes severe before seeking treatment
  4. Active riding during recovery: Mild Achilles tendon or IT Band issues usually do not require complete cessation of riding. Simply reduce volume (to 50% of normal training volume) and completely avoid climbing, allowing the tissue to continue adapting under low load
