The Complete Guide to Knee Injuries for Cyclists
Cycling is recommended by many doctors for patients with joint issues because it places far less impact on the joints than running. However, incorrect bike fit and training methods can still cause knee injuries. In fact, the knee is the most commonly injured area for cyclists, with approximately 65% of cyclists having experienced knee discomfort.
Three Main Types of Knee Injuries
1. Anterior Knee Pain
Location: Front of the kneecap (patella) or around it
Most Common Diagnoses:
- Patellar Tendinitis (Jumper’s Knee): Pain after riding
- Patellofemoral Pain Syndrome: Pain during riding or after descending
- Iliotibial Band Syndrome (anterior aspect): Less common
Common Causes:
- Saddle too low (excessive knee flexion angle, increasing patellar pressure)
- Cadence too low (using too heavy a gear)
- Weak core, knee valgus

- Raise the saddle to the correct height
- Increase cadence (target 85-95 rpm)
- Strengthen the glutes and quadriceps
2. Lateral Knee Pain
Location: Outside of the knee
Most Common Diagnosis:
- Iliotibial Band Friction Syndrome (IT Band Syndrome): One of the most common knee problems among road cyclists
Symptom Characteristics:
- Appears after approximately 40-60 minutes of riding
- Particularly noticeable when descending
- Subsides after stopping, returns upon resuming
Common Causes:
- Saddle too high (excessive leg extension at the bottom of the pedal stroke)
- Inappropriate pedal spacing (Q Factor)
- Improper cleat angle settings
- Weak gluteus medius
Solutions:
- Lower the saddle height (if too high)
- Adjust cleat angle to allow the foot to relax naturally
- Foam roll the iliotibial band (not recommended to massage directly when pain is severe)
- Gluteus medius strengthening: side steps, clamshells
3. Posterior Knee Pain
Location: Back of the knee (popliteal fossa area)
Most Common Diagnoses:
- Popliteus Tendinitis: Posterolateral knee
- Gastrocnemius origin issues: Posteromedial pain
- Meniscus problems: Various locations, requires medical diagnosis
Common Causes:
- Saddle too high (excessive ankle plantarflexion at the bottom of the pedal stroke, pulling on the posterior tendons)
- Heel dropping during the pedal stroke (increasing posterior tension)
Solutions:
- Lower the saddle height (adjust no more than 3mm at a time)
- Maintain ankle angle during the pedal stroke (avoid excessive plantarflexion)

Pedaling Technique
Knee Alignment:
- When pedaling, the knee should align with the direction of the second toe
- The knee should not cave inward (Valgus) or bow outward (Varus)
Ankle Angle:
- Keep the ankle neutral (avoid excessive dorsiflexion or plantarflexion)
- Pedal in circles (rather than just pushing down)
Cleat Setup
Cleat setup is crucial for knee health:
Fore-Aft Position: The cleat should be positioned directly above the pedal bolt, corresponding to the midpoint of the line connecting the first and fifth metatarsophalangeal joints
Float:
- It is recommended to use cleat systems with float (such as Shimano SPD-SL yellow cleats, which offer 6 degrees of float)
- Float allows the foot to rotate naturally, reducing torsional stress on the knee

Essential Preventive Exercises
Single Leg Squat:

- If the knee visibly caves inward during the movement, it indicates weak glutes
Glute Bridge:
- Strengthens the gluteus maximus, reducing knee compensation
Clam Shell:
- Strengthens the gluteus medius, improving knee stability
Straight Leg Raise (SLR):
- Strengthens the quadriceps, particularly the VMO (vastus medialis obliquus)
When to Seek Medical Attention
Stop riding and seek medical care if you experience:
- Knee swelling or effusion
- Inability to fully bend or straighten the knee
- Significant pain even when walking
- New symptoms following a traumatic injury
