Cycling is considered a low-impact sport, but riders with high training volume still face a fairly high risk of overuse injuries, and tendinitis is the most common type among them.
Understanding the causes, prevention, and recovery methods for these injuries is essential knowledge for any serious cyclist.
Three Common Tendon Problems
1. Patellar Tendinitis
Location: Below the kneecap (patella)
Symptoms:
- Pinpoint pain at the front-lower part of the knee
- Pain at the start of a ride, improving after warming up, then returning after the ride
- Pain when climbing or descending stairs
Most common causes:
- Saddle height too low (excessive knee flexion)
- Sudden increase in training volume
- Riding in too heavy a gear (pushing hard on the pedals)
2. IT Band Syndrome (ITBS)

3. Achilles Tendinitis
Location: Behind the heel

- Pain in the Achilles area while pedaling
- The Achilles tendon may feel swollen or thickened

- Heel dropped too low while pedaling (excessive ankle extension)
- Saddle too high
- Sudden increase in training volume
The Importance of Diagnosis
Tendinitis symptoms can easily be confused with other injuries. Before attempting self-directed recovery, it is recommended to consult a physical therapist or sports medicine physician, especially when:
- Pain has not improved after 1-2 weeks
- Pain persists even after rest
- Swelling appears
- There is a tearing sensation or a sudden worsening of pain
Prevention Strategies
Bike Fitting: The Most Important Preventive Measure
Many tendinitis problems originate from incorrect bike setup.
Effects of saddle height:
- Too low: increases the knee flexion angle, placing greater stress on the patellar tendon
- Too high: causes the leg to overextend, increasing IT Band tension and stressing the Achilles tendon
Recommended saddle height setup:
With the pedal at the lowest point (6 o’clock position) and the heel resting on the pedal, the leg should be almost fully extended, leaving about a 5-degree slight bend.
Gradual Increase in Training Volume
The “10% rule”: weekly training volume should not increase by more than 10% over the previous week.

Gear Selection
Use a slightly lighter gear (higher cadence) to reduce the joint load on each pedal stroke:

- Use the lightest suitable gear when climbing
Recovery Strategies
Acute Phase (1-2 weeks after injury)
The RICE Principle:
- Rest: Reduce training volume and avoid activities that worsen the pain
- Ice: Ice for 15-20 minutes after training (avoid direct contact with the skin)
- Compression: Reduce swelling
- Elevation: Helps reduce swelling
Principle for continuing to ride:
Mild pain (below 3/10) that does not worsen during riding allows for continued light riding.
Pain above 4/10, or pain that worsens during riding, requires complete rest.
Subacute Phase (2-6 weeks)
Introduction of Eccentric Training:
Eccentric training is currently the most strongly evidence-supported training method for tendinitis recovery, particularly for the Achilles and patellar tendons.
Achilles eccentric training (standing on the edge of a step):
- Rise onto both toes (concentric)
- Shift weight to the affected leg and slowly lower down (eccentric, 3-4 seconds)
- Repeat 15 times, 2-3 sets per day
- Expect a moderate degree of discomfort (this is normal)
Patellar eccentric training (decline squat):
- Use a 25-degree decline board (or a mat to create the incline)
- Perform slow squats (eccentric phase)
- The affected leg bears the primary load
Recovery Phase (6 weeks to 3 months)
Gradually return to normal training volume:
- Start at 50% of original training volume
- Increase by 10-15% per week
- Monitor symptoms and reduce intensity immediately if they recur
Special Handling for IT Band Issues
Recovery from ITBS requires particular attention to strengthening the core and glutes, since pelvic stability is one of the main causes of ITBS.
Gluteus Medius Strengthening Exercises
Clam Shell:
Lie on your side with knees bent at 90 degrees. Slowly lift the upper knee (without moving the pelvis), hold for 3 seconds, then lower. 3 sets of 15 reps daily.
Single-Leg Bridge:
Lie on your back with one foot on the floor and the other leg extended. Use the grounded foot and hips to lift the pelvis and hold for 5 seconds. 3 sets of 10 reps per side daily.
IT Band Stretching
Research on the effectiveness of direct IT Band stretching is limited, but the following method may help:
Standing IT Band Stretch:
Cross the affected leg behind the healthy leg and lean toward the affected side. Hold for 30 seconds, 3 times a day.
When to See a Doctor
Seek immediate medical attention in the following situations:
- Pain is very severe
- Extensive swelling appears
- You feel something has torn (possibly a tendon tear)
- No improvement after 6 weeks of conservative treatment
The value of physical therapy:
For most cases of tendinitis, intervention by a physical therapist can significantly shorten recovery time, provide a personalized training plan, and rule out other possible issues.
