IRONMAN Injury Prevention: The Three Achilles' Heels of Swimmer's Shoulder, Cyclist's Knee, and Runner's Foot

賽事分析

Introduction

The biggest enemy of IRONMAN training is not time or willpower, but injury. Once injured, 36 weeks of training can be reduced to nothing. This article provides a complete guide to prevention, correction, and rehabilitation for the most common injury in each of the three disciplines: swimmer’s shoulder, cyclist’s knee, and runner’s foot.

Swimmer’s Shoulder

  • Causes: High-frequency freestyle, rotator cuff overuse, hand crossing the midline during the pull, premature elbow lift
  • Symptoms: Anterior shoulder pain, pain when raising the arm overhead, waking up at night from pain
  • Prevention: YTWL × 3 sets per week, high-elbow catch, bilateral breathing, resistance band warm-up
  • Rehabilitation: Acute phase: ice and rest for 7-14 days → Sub-acute: elastic band ER/IR → Return: start with a 200m slow swim test

Cyclist’s Knee

Common types: Patellofemoral pain syndrome (PFPS) at the front of the knee, ITBS at the outer knee, pes anserine tendinitis at the inner knee.

  • Causes: Cleat position, incorrect saddle height, excessive Q-factor, too much heavy-gear riding, sudden increase in training volume
  • Prevention: Professional bike fit (NT$5,000-10,000), cadence of 85-95 rpm, quadriceps and gluteal strength, low-gear warm-up
  • Rehabilitation: Acute phase: ice and anti-inflammation → Foam roller for ITB → Short, flat, slow rides

Runner’s Foot

Common injuries: plantar fasciitis, Achilles tendinitis, tibial stress fractures, metatarsal stress fractures.

  • Causes: Sudden increase in mileage > 10%/week, running shoes older than 700km, insufficient ankle mobility, insufficient calf strength
  • Prevention: Rotate 2-3 pairs of shoes, supplement calcium and vitamin D, calf raises × 3 sets × 20 reps, arch training
  • Rehabilitation: Acute phase: ice and splinting → Golf ball rolling under the foot → Custom insoles → Progressive walking to jogging

Common Prevention Strategies for All Three Disciplines

Strategy Frequency Duration
Dynamic warm-up Before every training session 10-15 min
Static stretching After every training session 10 min
Foam rolling / massage 3 times per week 20-30 min
Strength training 2-3 times per week 30-45 min
Physical therapy consultation Once per month 60 min
Full sleep Every day 8 hours

When Should You Stop Training?

Red flags (stop training immediately and seek medical attention):

  • Joint swelling while running
  • Pain lasting more than 24 hours after training
  • Pain at a fixed point that progressively worsens
  • Pain affecting daily activities
  • Waking up at night from pain

Practical Advice

  • Invest in a physical therapist: consult once a month
  • Get sports injury insurance
  • Find a physician with sports medicine experience
  • Learn self-assessment: simple daily tests
  • Don’t push through pain: a minor injury costing 1 week is far better than a major injury costing 6 weeks

Conclusion

IRONMAN injuries are not “bad luck” but “accumulated results.” Investing 30-60 minutes per week in prevention (strength, stretching, foam rolling) is better than spending 6-8 weeks on rehabilitation after an injury. Remember: the key to your “finish rate” is not your fitness, but being able to “stand on the starting line.” Treat prevention as part of your training, and you can have a long-term IRONMAN career.
