Introduction
From childhood onward, every basketball player has heard the four words “Rest, Ice, Compression, Elevation” (RICE). But sports medicine has long moved beyond simply using RICE. In 2019, the British Journal of Sports Medicine proposed PEACE & LOVE, representing the evolution of acute injury management concepts.
This article compares the three major principles and explains why “excessive rest and icing” may actually slow down recovery.
Evolution of the Three Principles
| Principle | Era | Core Concept |
|---|---|---|
| RICE | 1978 | Complete rest, ice for anti-inflammation |
| MEAT | Post-2010 | Early movement, promote healing |
| PEACE & LOVE | 2019 | Integrates acute and sub-acute phases, includes psychological aspects |
RICE Explained
- Rest
- Ice
- Compression
- Elevation
Advantages: Easy to remember.
Disadvantages: Complete rest may delay healing; excessive icing may suppress tendon remodeling.
MEAT Explained
- Movement
- Exercise
- Analgesics (pain relief rather than anti-inflammation)
- Treatment
Primarily applied to tendon and ligament injuries, emphasizing that “tissues need appropriate stress to remodel.”
PEACE & LOVE Explained
Acute phase PEACE:
- P (Protection): Protect to avoid re-injury
- E (Elevation): Elevate to reduce swelling
- A (Avoid anti-inflammatories): Avoid suppressing healing
- C (Compression): Compression
- E (Education): Educate on self-management
Sub-acute phase LOVE:
- L (Load): Progressive loading
- O (Optimism): Stay optimistic
- V (Vascularization): Maintain cardiovascular aerobic activity
- E (Exercise): Progressive exercise
When to Ice?
- Within the acute 24–48 hours: 10–15 minutes every 2 hours
- Primary purpose: pain relief rather than “anti-inflammation”
- No routine icing after that period
- Chronic tendinopathy: brief icing after training for pain relief
When to Use NSAIDs?
- Acute pain affecting sleep: short-term 3–5 days
- Chronic tendinopathy: not recommended
- Acute phase of fractures or muscle injuries: studies suggest they may delay healing
Practical Comparison Table
| Injury | Recommended Principle |
|---|---|
| Acute ankle sprain Grade I | PEACE & LOVE |
| Muscle strain | PEACE & LOVE |
| Chronic tendinopathy | MEAT + progressive loading |
| Acute fracture (conservative) | Protection + progressive LOVE |
| Post-operative ligament tear | Individual protocol |
Practical Advice
- Don’t “over-rest” and stay immobile for an entire week
- Use compression bandages correctly
- Elevate 20–30 cm above heart level
- Early ROM is better than complete immobilization
- The psychological aspect is equally important; avoid fear of movement
- Basketball players can still maintain upper-limb and cardiovascular training after injury
- National Taiwan University of Sport and Chang Gung Sports Medicine Center have adopted the PEACE & LOVE concept
Conclusion
Sports medicine continues to evolve. Please update your knowledge and stop rigidly following the 1970s RICE approach. PEACE & LOVE combines “protecting healing” with “active recovery” and is the best framework for contemporary acute injury management. Next time you’re injured, remember: more rest is not always better, and earlier movement is not always better—balance is the key.
