Cycling Guidelines for Patients with Benign Prostatic Hyperplasia (BPH)
Benign prostatic hyperplasia (BPH) has a prevalence of approximately 40–50% among Taiwanese men over 50, and 50–60% among those over 60. For BPH patients, cycling is not contraindicated, but special attention is required:
- Monitor urination before and after long rides: If urine flow becomes noticeably weaker or a sensation of urinary retention occurs, stop riding and seek medical attention
- Avoid cycling during acute exacerbation of symptoms: If frequent urination or worsening nocturia has already developed, treat and control symptoms first
- Stop riding 48 hours before PSA testing: Ensure PSA test results are not affected by cycling-related stimulation
- Patients taking alpha-blockers should watch for hypotension: Some BPH medications may cause orthostatic hypotension after exercise
Practical Recommendations
Daily preventive habits:
- After rides exceeding 1 hour, take a warm bath (38–40°C) for 15 minutes to help restore blood circulation around the prostate
- Maintain adequate fluid intake, drinking at least 500 mL of water per hour while riding to support normal prostatic fluid secretion
- Within 48–72 hours after long endurance events, avoid intense core-compressing abdominal exercises to allow the pelvic floor muscles to fully relax
Regular health screenings:
- Cyclists over 40 are advised to undergo annual PSA testing (remember to have blood drawn 48 hours after riding)
- If there is a family history of prostate problems, screening is recommended starting at age 35
- Annual urology check-ups can be combined with sexual function assessment, as the impact of perineal compression on erectile function sometimes appears earlier than effects on the prostate itself
