
Introduction
Have you noticed that as your training volume continues to accumulate, your running pace has plateaued or even declined? It’s not that you’re regressing—you may have fallen into the trap of “runner’s anemia.” Sports Anemia affects approximately 15–35% of endurance runners, and up to 50% of female runners. It’s a health issue that severely impacts training effectiveness yet is often overlooked.
The Role of Iron in Running
Iron is the core element in synthesizing hemoglobin, which is responsible for carrying oxygen to working muscles. Iron deficiency directly reduces maximal oxygen uptake (VO₂max), and even if all other training conditions remain unchanged, running performance will decline significantly.
Three Mechanisms by Which Running Depletes Iron

Special note: “Dilutional Pseudoanemia” is not true iron deficiency but rather a result of training adaptation. The absolute amount of hemoglobin has not decreased, and iron supplementation is not needed. This is an important concept that requires careful differentiation in runner’s anemia.
Identifying Symptoms
The symptoms of iron-deficiency anemia are often gradual and difficult to detect in the early stages:
- Early stage (iron stores depleted but no anemia yet): No specific symptoms, but slower recovery from training
- Middle stage (iron-deficiency anemia): Easy fatigue, declining endurance, slow heart-rate recovery after training
- Overt anemia: Elevated resting heart rate, shortness of breath after mild activity, dizziness, pale complexion, poor concentration, brittle hair (these symptoms are sometimes mistaken for “overwork” rather than anemia among women in Taiwan)
Diagnosis and Monitoring
It is recommended to have regular blood tests every six months to check the following markers:

Key point: “Iron Depletion without Anemia”—where hemoglobin is normal but ferritin is < 20 ng/mL—can also impair athletic performance and requires proactive iron supplementation.
Dietary Iron Strategies—Local Taiwanese Ingredients
Recommended High-Iron Foods
Heme Iron (absorption rate 15–35%), from animal sources:
- Pork liver: approximately 23mg of iron per 100g (can be eaten 1–2 times per week)
- Clams: approximately 14mg of iron per 100g (abundant in Taiwan, affordable and high quality)
- Beef shank: approximately 3–4mg of iron per 100g
- Tuna/dried bonito flakes: approximately 2–3mg of iron per 100g
Non-Heme Iron (absorption rate 2–20%), from plant sources:
- Red beans, black beans: approximately 7–9mg per 100g (pairing with vitamin C can enhance absorption)
- Red amaranth, spinach: approximately 3–5mg per 100g
- Tofu (firm tofu): approximately 2–3mg per 100g
- Red dates: approximately 2mg per 100g (commonly used as a blood-nourishing food in folk tradition, but iron content is not as high as commonly believed)
Tips for Improving Iron Absorption
- Consume vitamin C simultaneously: Orange juice, guava, and guava (abundant in Taiwan) eaten together with iron-rich foods can increase non-heme iron absorption by 2–3 times
- Avoid drinking tea or coffee at the same time: Tea polyphenols and caffeine inhibit iron absorption; wait at least 1 hour after meals before drinking
- Reduce antacid use: Long-term users of PPIs (stomach medication) have impaired iron absorption
- Do not consume milk with iron-rich foods: Calcium competitively inhibits iron absorption
Iron Supplementation Considerations
If serum ferritin remains low after dietary adjustments, use iron supplements under a physician’s guidance:
- Common oral iron supplements (such as ferrous sulfate) are recommended to be taken on an empty stomach for best results, but may cause gastrointestinal discomfort as a side effect
- Those with sensitive stomachs can switch to taking them after meals or switch to ferrous gluconate (gentler)
- Do not over-supplement on your own: iron overload carries a risk of liver toxicity; dosage should be adjusted based on blood test results
Conclusion
Runner’s anemia is a problem that “silently” undermines performance—unlike a sprain, it offers no clear pain warning. Regular blood testing is a low-cost investment to protect your running performance, especially for female runners, vegetarian runners, and those running more than 200km per month. These runners need to proactively monitor their iron status so that every kilometer of training can be effectively converted into real progress.
