One of the most common frustrations on Taiwan’s road racing scene is side stitch (Side Stitch, formally known as Exercise-Related Transient Abdominal Pain, ETAP). Many runners blame their pre-race diet, yet overlook the possibility that breathing cadence may be a more direct cause. Running breathing is not merely about “taking a few more breaths”—cadence, depth, and body coordination all have a scientific basis.

Basic Concepts of Breathing Cadence

Breathing cadence (Breathing Rhythm or Breathing Cadence) refers to how many steps are taken across the inhalation and exhalation phases. The two most common cadences are:

  • 2:2 cadence: Inhale for 2 steps, exhale for 2 steps, completing one full breathing cycle every 4 steps. This is the cadence most runners naturally adopt at moderate to high intensity.
  • 3:2 cadence: Inhale for 3 steps, exhale for 2 steps, completing one cycle every 5 steps. Popularized by running coach Budd Coates, its defining feature is that each exhalation lands on a different foot, creating an “asymmetrical cadence.”

There are also faster cadences like 2:1 (high-intensity sprinting) or slower ones like 4:4 (easy warm-up), adjusted according to intensity.

Characteristics of the 2:2 Cadence

The advantage of 2:2 lies in its intuitive simplicity—it syncs easily with stride frequency and meets ventilatory demands during moderate-intensity running at a heart rate of 150–170 bpm. However, the problem with 2:2 is:

  • Exhalation consistently occurs on the same side’s foot strike (assuming each cycle starts with the right foot), causing the diaphragm on that side to bear sustained greater pressure.
  • Research indicates that during exhalation, the diaphragm relaxes and the viscera shift downward; if the same-side foot strikes simultaneously, the downward traction on the liver (right side) or spleen (left side) pulls against the upward force of the diaphragm—a possible mechanism behind side stitch (Kriz & Martin, 2020).

Advantages of the 3:2 Asymmetrical Cadence

With the 3:2 cadence, each exhalation alternates between the left and right foot, so the same side never continuously bears the dual stress of ground impact plus exhalation:

Physiological reasons supporting 3:2:

  • Balanced diaphragm pressure on both sides: avoids chronic overload on one side.
  • Steady cadence and parasympathetic activation: the regularity of the 5-step cycle helps maintain a relaxed state and reduces race anxiety.
  • Deeper breathing: inhaling over 3 steps means a longer inhalation time per breath, facilitating diaphragmatic breathing expansion.

Breathing Depth: Chest Breathing vs. Belly Breathing

Regardless of cadence, belly (diaphragmatic) breathing takes in more air than chest breathing:

  • Belly breathing: the diaphragm descends, the abdomen expands outward, and the lower lungs fully expand, with a tidal volume of approximately 500–700 mL per breath.
  • Chest breathing: the intercostal muscles lift the rib cage, the lower lungs are underutilized, tidal volume is smaller, and more respiratory muscle energy is consumed.

Training method: lie flat with one hand on your chest and the other on your abdomen; practice letting your abdomen rise first during inhalation, followed by the chest, then let the abdomen sink during exhalation. This can be easily practiced in the warm-up area at Taipei Dajia Riverside Park.

Matching Breathing Cadence to Pace: Recommendations

  1. For those prone to side stitch: prioritize switching to the 3:2 cadence and evaluate the results after 3–4 consecutive weeks of training.
  2. For beginners: first establish a steady sense of cadence with 2:2, then attempt the 5-step cycle of 3:2.
  3. During pre-race warm-up: start the diaphragm with 5–10 minutes of belly-breathing jogging to reduce the likelihood of sudden breathing disruption during the race.
  4. During high-intensity intervals: do not force a specific cadence; let your body self-regulate, and only focus on maintaining belly-breathing depth.
