The Anterior Cruciate Ligament (ACL) is located in the center of the knee joint, connecting the femur to the tibia, and its primary role is to prevent the tibia from sliding too far forward. For a sport like basketball that requires sudden stops, changes of direction, and landings, the ACL acts as the knee’s “fuse”—once it tears, it almost certainly ends the season. From the NBA to the SBL, UBA, and HBL, this injury takes away key players every year.

Practical Recommendations

  • If you hear a “pop” at the moment of injury and the knee swells immediately, assume it is an ACL tear and stop exercising
  • Do not attempt to walk it off or test it yourself, to avoid concurrent injuries (meniscus, cartilage)
  • Apply RICE within the golden 48 hours and schedule an orthopedic sports medicine clinic as soon as possible
  • Surgery does not need to happen within the first week; restoring ROM and quadriceps strength first can reduce the risk of joint stiffness
  • For rehabilitation, be sure to choose a physical therapist experienced in ACL recovery; the NTNU Sports Science Center and Chang Gung Sports Medicine Center both have complete protocols
