Hamstring Strain: An Analysis of Explosive Power Injuries in Basketball Players

Introduction

Hamstring strains are a signature injury in track and field and soccer, but basketball players are equally susceptible during fast-break transitions, sudden stops, and explosive drives to the basket. The hallmark of this injury is “quick to injure, slow to recover, and prone to recurrence.”

This article is based on the British Athletics Muscle Injury Classification (BAMIC), covering mechanisms, diagnosis, rehabilitation, and return-to-play criteria.

Anatomy and Classification

The hamstring is composed of three muscles:

  • Semitendinosus
  • Semimembranosus
  • Biceps femoris

The long head of the biceps femoris is the most commonly injured, accounting for approximately 80% of hamstring strains in basketball players.

Assessment

  • H-test: resisted hip extension with knee flexion
  • Askling H-test: simulates eccentric contraction
  • Ultrasound/MRI to confirm grade

Core Rehabilitation: The Askling Protocol

Includes three key exercises:

  1. The Extender: supine with hip flexed to 90 degrees, slowly extend the knee to the point of pain
  2. The Diver: standing single-leg forward bend, simulating a catching posture
  3. The Glider: sliding movement with controlled stretching

Perform 3–6 reps per set, 1–2 times daily, starting from sub-pain levels.

Rehabilitation Timeline

Practical Recommendations

  • Stop immediately at the moment of injury; do not try to “run it off”
  • Do not stretch within the first 7 days to avoid delaying healing
  • Recurrence rate is as high as 30–50%; complete the full rehabilitation program
  • Nordic curls are the gold-standard preventive exercise
  • Pre-game warm-up with RAMP (Raise, Activate, Mobilize, Potentiate) is essential

Conclusion

The biggest trap with hamstring strains is returning to play “as soon as it feels better.” Research shows that most recurrences occur within two weeks of returning to play. Be sure to pass all objective criteria before returning, and maintain Nordic curls once per week as prevention.
