“My hands go numb halfway through a ride,” “After getting off the bike, my toes take several minutes to regain feeling”—these symptoms are extremely common among cyclists in Taiwan, yet they are often dismissed as normal and ignored. In fact, hand and foot numbness caused by cycling reflects real nerve compression or peripheral circulatory disorders, and if left unaddressed over the long term, may develop into chronic nerve damage.

Neuroanatomy of Hand Numbness

The weight and vibration of the hands pressing on the handlebars over long periods primarily affect two nerves:

Immediate Assessment of Hand Numbness

  • Which fingers feel numb? Ulnar side (little finger side) → ulnar nerve; radial side (thumb side) → median nerve
  • How long does the numbness take to subside after getting off the bike? Disappears within 10 minutes → temporary compression; lasts more than 1 hour → medical evaluation needed
  • Is there accompanying muscle weakness? For example, inability to abduct the little finger → warning sign of nerve function impairment

Analysis of Foot Numbness Causes

Numbness in the toes or soles of the feet is also common during long rides, and the causes can be divided into two main categories:

Mechanical Compression

Tight cycling shoes are the most direct cause. During cycling, the feet swell slightly due to exertion. If the shoe’s last is narrow or the straps are too tight, the interdigital nerves in the forefoot become pinched, causing a Morton’s neuroma-like tingling pain, usually concentrated in the 2nd–4th toes.

Pedal Contact Point Issues

  • Cleat position too far forward: The forefoot bears pedaling force for extended periods, compressing the nerve plexus beneath the metatarsal heads
  • Insufficient sole stiffness: Foot flexion causes repeated compression of the midfoot nerves
  • Misaligned pedal spindle position: If the ball of the foot (first metatarsal head) is not aligned with the pedal spindle center, abnormal biomechanics increase nerve tension

Adjustment Strategies: From Posture to Equipment

Methods to improve hand numbness:

  • Handlebar height: Raise the handlebars (move stem spacers up or use a shorter-drop stem) to shift the body’s center of gravity rearward, reducing the proportion of weight borne by the hands. It is recommended to keep the torso angle at 40–45°, avoiding excessive forward lean
  • Handlebar width: Handlebar width should be close to your shoulder width (acromion-to-acromion distance). Too narrow a bar restricts shoulder rotation and increases wrist compensation
  • Grip material: Switch to silicone bar tape with better vibration absorption or thicker tape to reduce direct transmission of road vibration to the nerves
  • Glove selection: Choose cycling gloves with gel padding at the heel of the palm, and change hand positions every 45–60 minutes (from the hoods to the drops, or hooking fingers over the bar end)
  • Wrist posture: Keep the wrist in a neutral position, avoiding excessive extension or flexion. Ideally, the wrist and forearm should be nearly in a straight line

Methods to improve foot numbness:

  • Shoe size: It is recommended to go half a size larger than your everyday shoes, and re-check strap tightness midway through long rides
  • Cleat position: Adjust the cleat position so the ball of the foot aligns with the pedal spindle center, avoiding positioning too far forward
  • Pedal selection: Ensure the pedal platform is large enough to support the forefoot; too small a contact area concentrates pressure
  • Insole upgrade: Use custom or semi-custom orthotic insoles, especially for riders with flat feet or high arches, to improve arch support and reduce metatarsal pressure

Practical Recommendations

Active prevention during rides:

  • Every 20–30 minutes, actively relax the fingers by alternating between making a fist and spreading the fingers 10 times
  • During long rides, periodically perform small circular movements with the wrists
  • On climbs or gentle slopes, try standing pedaling to give the hands brief relief from pressure
  • Regularly change hand positions; road cyclists should make good use of rotating among the hoods, drops, and tops of the brake levers

Core strengthening beyond training:

The root cause of hand numbness is often insufficient core strength, causing excessive weight to be borne by the arms. Strengthening the transversus abdominis and multifidus muscles through core training helps the upper body “float” over the handlebars rather than pressing down on them.

Medical consultation recommendations:

If the following symptoms occur, it is recommended to see a neurologist or hand surgeon:

  • Hand numbness lasting more than 24 hours
  • Obvious muscle atrophy in the fingers
  • Inability to perform fine motor tasks (such as buttoning a shirt)
  • Symptoms also appear at night or when not riding
