Iron in Female Athletes: The Dual Drain of Menstrual Blood Loss and Endurance Exercise

Why Are Female Athletes Especially Prone to Iron Deficiency?

Between iron intake and loss, female athletes face a dual burden:

Route of Loss General Female Population Female Endurance Athletes
Menstrual blood loss 5–15 mg/month Same as left, but higher with irregular cycles
Sweat 0.3 mg/L 0.5–1 mg/L × heavy training volume
Gastrointestinal (GI bleeding) Minimal Running impact can reach 1–2 mg/day
Red blood cell destruction (foot strike) None Significant hemolysis from foot-strike impact in runners
Total Loss ~1 mg/day 2–4 mg/day

The recommended intake for women is 18 mg/day, endurance athletes should aim for 20–25 mg/day, and vegan female athletes need 30+ mg/day.

The Three Stages of Iron Deficiency

Stage Ferritin (μg/L) Hb (g/dL) Symptoms
Iron store depletion < 35 Normal No obvious symptoms, but performance begins to decline
Iron-deficient erythropoiesis < 20 12–13 Fatigue, slow recovery after training
Iron deficiency anemia < 12 < 12 Dizziness, palpitations, significant performance decline

Key Point: Ferritin levels in female athletes should be maintained above 30–50 μg/L. The lower limit under Taiwan’s National Health Insurance standards (10–15) is too low for athletes.

Specific Data on Performance Impact

  • Ferritin < 20 μg/L: VO2max drops by an average of 4–8%
  • Ferritin < 12 μg/L: 5K performance drops by 8–12%
  • Endurance training adaptations nearly stall (mitochondrial biogenesis requires iron)

Dietary Strategies for Iron Intake

Heme Iron (absorption rate 15–35%)

  • Red meat (beef, lamb): 3 oz contains 2–3 mg
  • Liver: 100g contains 6–9 mg (most potent per serving)
  • Clams: 100g contains 28 mg (champion among foods)
  • Egg yolk: 1 yolk contains 0.9 mg

Non-heme Iron (absorption rate 2–10%)

  • Spinach (cooked): 1 cup 6.4 mg
  • Red lentils: 1 cup cooked 6.6 mg
  • Black sesame seeds: 30g contains 4.5 mg
  • 70% dark chocolate: 30g contains 3.4 mg

Enhancing Absorption

  • Consume with Vitamin C: increases non-heme iron absorption by 3–4 times (orange juice + spinach)
  • Avoid inhibitors: separate tea, coffee, and calcium supplements from iron-containing foods by more than 2 hours

Should You Take Iron Supplements?

Get a blood test before supplementing. Blind iron supplementation may cause:

  • Gastrointestinal discomfort, constipation
  • Hemochromatosis
  • Interference with zinc and copper absorption

Supplementation Recommendations (after confirmed deficiency)

  • Alternate-day dosing is more effective than daily dosing (avoids hepcidin elevation blocking absorption)
  • Take within 30 minutes after training: avoids the post-exercise hepcidin peak
  • Dosage: 60–100 mg elemental iron / every other day
  • Forms: ferrous sulfate, ferrous gluconate, iron bisglycinate (better absorption and less constipation)
  • Re-test after 8–12 weeks of supplementation

Monitoring Frequency

  • General female athletes: full iron panel once a year
  • High training volume (>10 h/week): every 6 months
  • Before the competitive season: mandatory
  • Vegans / those with heavy menstrual flow: every 3–4 months

A Frequently Overlooked Fact

Recovering ferritin from 12 back to 50 takes at least 3–4 months. Getting a blood test and supplementing just one month before the season is too late. Build ferritin monitoring into your annual training plan.
