“After swimming, my ear always feels like it has water in it, and a few days later it starts to hurt…” This is a familiar experience for many swimming enthusiasts in Taiwan. “Swimmer’s Ear,” medically known as Otitis Externa, is the most common ear problem among swimmers and a frequent visitor to ENT outpatient clinics during the summer.
Taiwan’s hot and humid summer climate, high pool usage rates, and the common Taiwanese habit of “cleaning the ears” with cotton swabs (which is ironically one of the biggest triggers for swimmer’s ear) all contribute to a relatively high incidence of swimmer’s ear. - Hearing aid users: Hearing aids block the ear canal for extended periods, reducing air circulation
- Those with allergic dermatitis or eczema: The skin barrier function of the ear canal is already weaker
- Individuals with diabetes: Weaker immune function may lead to more severe infections (malignant otitis externa is a serious complication in diabetic patients)
- Frequent swimmers (more than 5 times per week): The ear canal lacks sufficient recovery time
Effective Prevention Methods for Swimmer’s Ear
Post-Swim Ear Canal Drainage Techniques
The first step after getting out of the water is to help drain trapped water from the ear canal:
- Tilt-and-tap method: Tilt your head to one side, gently tap behind the same-side ear (not the ear canal opening) with your palm, using the vibration to help water drain; repeat on the other side.
- Single-leg hopping method: Tilt your head to one side and hop gently on the same-side leg, using gravity to accelerate drainage.
- Hairdryer-assisted drying: Use a hairdryer on the lowest “cool” or “low heat” setting, held 20-30 cm from the ear canal, blowing for 30-60 seconds to help evaporate residual moisture. Do not use high heat or hold it too close to avoid burns.
- Swimming-specific ear plugs: Using silicone ear plugs shaped to fit the ear canal (not foam ear plugs) while swimming is the most direct preventive measure.
Use of Diluted Acetic Acid Solution
ENT physicians often recommend instilling 2-3 drops of a diluted acetic acid solution (white vinegar mixed with an equal amount of water, i.e., a 1:1 dilution) into the ear canal after swimming, tilting the head to let it sit for 30 seconds, then allowing it to flow out naturally. The weak acidity of acetic acid restores the ear canal’s pH level and inhibits bacterial growth, making it a safe and effective preventive measure.
Note: If there is a perforated eardrum, or if wounds are already present in the ear canal, absolutely do not use acetic acid solution (it may cause severe pain and further damage). Please confirm the eardrum is intact before use.
Choosing Waterproof Swimming Ear Plugs
There are various swimming ear plugs available on the Taiwanese market:
- Silicone ear plugs: The most common type, reusable, with good waterproofing; the downside is they may not fit perfectly due to different ear canal shapes
- Wax ear plugs: Mold to the shape of the ear canal, providing a high level of seal; suitable for those at high risk of swimmer’s ear, but not reusable
- Custom-molded silicone ear plugs: Made by audiologists or ENT specialists, fitting the individual’s ear canal perfectly; more expensive but offer the best results, suitable for professional swimmers or frequent swimmers
Behaviors to Completely Avoid
- Do not clean the ear canal with cotton swabs: The American Academy of Otolaryngology explicitly recommends that no one (including non-swimmers) should use cotton swabs to clean inside the ear canal, as this disrupts the natural protective mechanism and creates micro-wounds.
- Do not dig in the ears with fingers or other sharp objects: For the same reason.
- Do not continue swimming during the acute phase of swimmer’s ear: The waterlogged ear canal needs time to dry and recover; forcing continued swimming will only worsen the infection.
Home Management of Swimmer’s Ear and When to Seek Medical Attention
Home Management for Mild Swimmer’s Ear
If symptoms are mild (only itching or slight discomfort), you can try the following at home:
- Thoroughly keep the ear canal dry and stop swimming for 2-3 days
- Use a diluted acetic acid solution (provided the eardrum is confirmed to be intact)
- Avoid using cotton swabs and let the ear canal recover naturally
Situations Requiring Immediate Medical Attention
The following situations require a visit to an ENT specialist without delay:
- Ear pain lasting more than 24 hours that has not improved or is worsening
- Yellow or green discharge from the ear canal
- Visible redness and swelling of the ear canal, or even spreading to the skin around the auricle
- Noticeable hearing loss
- Accompanied by fever (body temperature exceeding 38°C)
- Patients with diabetes or immune system issues should seek medical attention as soon as symptoms appear

Establish a fixed post-swim ear care routine: Perform ear canal drainage and drying immediately after getting out of the water every time, making this action as automatic as showering after a swim.

Choose pools with good water quality: Pools with strict water quality management (regular monitoring of bacterial counts, timely replenishment of disinfectants) have lower bacterial levels, reducing the risk of infection.

Avoid not rinsing after swimming in natural waters (streams, lakes, the sea): Natural waters contain a more diverse range of bacteria, so it is even more important to thoroughly rinse the ear canals with clean water after swimming.

Teach children proper ear care habits: The incidence of swimmer’s ear is higher in children (because children’s ear canals are narrower, making water drainage more difficult). Establishing the habit of tilting the head to drain water after swimming from a young age is much easier than changing habits in adulthood.
