Swimmer's Shoulder Rehab: 5 Complete Scapular Stabilization Exercises

The Scapula: The Overlooked Key

90% of swimmer’s shoulder patients don’t have a problem with the rotator cuff itself, but with poor scapular control (Scapular Dyskinesis). If the scapula cannot properly upwardly rotate and posteriorly tilt, the rotator cuff is forced to work at incorrect angles, eventually leading to inflammation.

The core of rehabilitation is training the four scapular muscles:

Five Scapular Stabilization Exercises

Exercise 1: Y-T-W-L Prone Floor Training

Target Muscles: Lower trapezius, rhomboids

  • Lie face down on the floor with a towel under your forehead
  • Position your arms sequentially into the letters Y, T, W, and L
  • Hold each position for 2 seconds, lower for 2 seconds
  • 10 reps per letter, 40 reps total per set
  • 2-3 sets daily

Key Point: When lifting, imagine “moving the scapula downward toward the spine,” not using arm strength.

Exercise 2: Wall Slides

Target Muscles: Serratus anterior, lower trapezius

  • Stand with your back against the wall, feet 30cm away from the wall
  • Keep elbows, wrists, head, upper back, and glutes against the wall
  • Slide both hands up the wall overhead, then slide back down
  • 10 reps × 3 sets
  • Keep your lower back against the wall throughout

Progression: Use a foam roller against the wall to add instability.

Exercise 3: Prone Dumbbell Raises (Prone T and Y)

Target Muscles: Middle and lower trapezius

  • Lie face down on a training bench, holding a 1-2 kg dumbbell in each hand
  • Raise your arms in a T position, palms facing down
  • Lift to shoulder height, hold for 2 seconds
  • 12 reps × 3 sets

Key Point: Err on the side of lighter weight; 2 kg is sufficient. Too much weight will cause compensation from the deltoids.

Exercise 4: Band External Rotation

Target Muscles: Infraspinatus, teres minor

  • Squeeze a towel between your elbow and your side, forearm at 90 degrees
  • Pull the resistance band outward from the inside
  • Externally rotate 30-45 degrees, hold for 2 seconds
  • 15 reps × 3 sets per side

Band Selection: Yellow or red (low resistance); the focus is on control, not weight.

Exercise 5: Prone Scapular Push-up Plus

Target Muscles: Serratus anterior

  • Start in a standard plank position
  • Without bending the elbows, only use the scapulae to “depress” and “push up”
  • When pushing up, imagine rounding your upper back into an arch
  • 15 reps × 3 sets

Progression: Once this becomes easy, progress to placing your hands on a BOSU ball or an unstable surface.

Common Mistakes

  1. Shrugging Compensation: Shoulders must not shrug in any exercise; imagine extending the collarbones outward.
  2. Excessive Weight: Better to do 15 perfect reps with 1 kg than 15 sloppy reps with 5 kg.
  3. Insufficient Reps: Scapular stabilizers are primarily slow-twitch muscles; 12-15 reps per set is required for effectiveness.
  4. Ignoring One Side: Even if only one side is painful, train both sides to avoid imbalance.

When to Return to the Water

Only resume full swimming volume after meeting the following criteria:

  • Complete all 5 exercises pain-free
  • Raise arms overhead without a painful arc
  • No spontaneous pain at night
  • Negative Empty Can test

Scapular stability is a lifelong task for swimmers; even without injury, maintain training twice a week.
